We are committed to enhancing the discharge process for hospitals by providing seamless and reliable nursing services. Our goal is to ensure a smooth transition from hospital to home, allowing patients to receive the care they need in the comfort of their own surroundings. We encourage health professionals to refer their patients to us, knowing that we will work closely with you to ensure a streamlined discharge and ongoing care process.

How to Refer a Patient

1. Download the Referral Form

Please download the referral form from the link provided below.

2. Complete and Submit the Form

After completing the form, please email it to referrals@inhomecarevic.com.au

3. Confirmation Process

Submitting the referral form does not immediately confirm a booking. Our team will review the form to assess the patient’s funding, eligibility, and the availability and capacity of our services in the required area.

4. Response Time

You can expect a response within the day to confirm the receipt of your referral. We will then contact the health facility to finalise and confirm the booking.
